The layout has no columns in it, so every label is the same. The starting number fills the rest of one sheet.
In the print dialog choose Actual size (100%). “Fit to page” shrinks the sheet and pushes every label off its cut. The PDF asks your viewer for actual size, but check before you print a full run.

Check the fit before you print on labels. Print the blank template on plain paper, hold it against a sheet of Avery 5164 up to the light, and check the boxes land on the cuts. Worth doing with any template on any printer: paper size, driver margins and scaling all shift a sheet. It matters more here: these measurements come from an open label-template database and are checked arithmetically on every build, and the same database reproduces this site’s paper-verified Avery 5160 layout exactly, but Avery 5164 itself has not yet been measured against real stock.

What is Avery 5164?
Avery 5164 is a US Letter sheet of six shipping labels, each three and a third inches tall and four inches wide, in two columns of three. The rows touch, with no gap between them, and the two columns are three-sixteenths of an inch apart.
It is a 5163 cut into three rows instead of five. The columns sit in exactly the same place, four inches wide with the same five-thirty-seconds side margin, and each label is three and a third inches tall instead of two. Six of them cover 80 square inches, the same label area as ten 5163 labels, divided into fewer, larger pieces.
What is Avery 5164 used for?
Avery 5164 is for parcels and cartons that need more on the label than an address. There is room for a sender block, a large recipient address, and an order number or a code on the same label.
It also works as a large product or box-contents label: a product name, a short description, a batch or date line, and a code underneath. With six to a sheet, a run of thirty parcels is five sheets, and a sheet left half-used from the last run can be finished by starting at its first unused label.
How do I print Avery 5164 labels from a spreadsheet?
To print Avery 5164 labels, add your list in the tool on this page: an Excel or CSV file, or cells pasted from Google Sheets. A US address list is laid out automatically, and any other column goes on the label by writing its name in braces, such as {Order} on a line of its own.
The layout takes up to eight lines, and on a 5164 all eight still fit at 18 point, so there is no need to squeeze. Under More options you can make the recipient's name larger than the rest, or print a Code 128 barcode or a QR code from one of the columns.
Should I use Avery 5164 or 5163?
Use Avery 5164 when each label has to carry a barcode or QR code as well as a full address, and the 5163 when an address and a reference line are enough. Both sheets hold the same total label area, so the difference is only how it is divided: ten labels of two by four inches, or six of three and a third by four.
The extra height matters most for codes. In barcode mode the 5164 starts from the same three-inch symbol width as the 5163, with bars 1.25 inches tall instead of 0.75, and a QR code sits centered at the top, almost two inches square, with the text underneath. Avery 5264, 8164 and 8464 have the same measurements, 8164 and 8464 being the inkjet versions, so a PDF made for one prints on the others.
